​INSURANCE   Our in-network contracts include Anthem, Anthem Medicaid, Med Mutual,  Aetna, United Health Care/ Optum, United Health Care Medicaid, CareSource, Medicaid, Buckeye, Paramount, and Medicare*.  Other insurance plans are accepted on an out-of-network basis.   We also accept self-pay.  Our fee schedule can be found on the Intake Forms page.   (*Not all clinicians may be credentialed with these plans. Medicare, Medicare Advantage, and other insurance holders are limited to those clinicians that are credentialed to submit claims to various commercial insurance plans and to Medicare.  Please ask before scheduling an appointment if your therapist is credentialed with your insurance.  Some services, such as Marriage Counseling, may not be covered by insurance.)
